Learning from Adverse Events Reporting
The national adverse events reporting policy sets out local organisational roles and the national role of Te Tāhū Hauora Health Quality & Safety Commission (Te Tāhū Hauora) in reporting, reviewing and learning from adverse events. Reporting serious adverse events nationally and regionally is an important part of improving health outcomes for New Zealanders. It improves safety and encourages open and transparent reporting of incidents when something goes wrong.
The Learning from Adverse Event report is used to support our continuous quality improvement strategy, focusing on shared learning to improve systems and minimise the possibility of future incidents.
