The clinical team encompasses clinicians and staff who work together with the practices.
This includes:
- Te Waka Ngākau o Poutini
- Nurse Practitioner and Kaiawhina
- Funded healthcare visits for high risk, unenrolled, and unengaged populations
- Youth specific clinics
- Aims to improve access and support re-engagement with primary care practices
- Quality Team
- Manages and implements clinical programme contracts (see Clinical Programmes Overview below)
- Reports quality metrics data received from the practices to HNZ
- Advocates for healthcare funding for the West Coast practices
- Connection and communications for all the practices on the coast
- Organises West Coast clinical education
- Equity Pharmacists
- In-practice work with patients and providers
- Outreach with community
- Prescriber auditing, quality improvement, and education
- Health Pathways Coordinator
- Manages West Coast specific pathways
- Assists with West Coast ERMS pathways
